- In order to best serve you, it is important to bring your insurance card, any pertinent documentation and primary physician referrals if needed to your office visits.

Payment Policy
- Routine Office Visits: Payments and co-payments for office visits are due at the time of service. Should unexpected medical expenses arise, our business manager will gladly assist you in making special arrangements.
- Obstetrical Office Visits: In order to assist you with your financial planning during the prenatal phase of your pregnancy, our business staff will review your insurance coverage and health care investment at your first obstetrical visit. Our policy for payment of obstetrical services is designed to help you plan for the new expenses of a growing family.
